Question: Is there a table (or anywhere else) that will display which terminal tried logging in that eventually locked an SAP user account?
Answer:
Use SM21 and search for the string "locked". When you drill down on the message, you'll see the network ID of the terminal where the illegal logon was initiated.

If a SAProuter is involved, I'm sure you'll only find the IP address and "terminal name" of the SAProuter.
The same is true if several users login via the same Windows based terminal server.
There's no chance to distinguish the individual PCs from within SAP.
